Christ's Church of the Valley (CCV) Waiver & Media Release

I, the undersigned, as the parent or legal guardian of the named child above (the “Child”), do hereby give my full consent and approval for the Child to participate in the named activity and assume all liability for risk or harm to the Child associated with such participation.

I understand that in the event of a medical emergency, every effort will be made to contact me based on the information provided above. In the event I cannot be reached, I hereby consent to the administration of such emergency medical care to the Child as deemed appropriate under the circumstances; including treatment by any physician, nurse, medical authority and/or hospital and transportation to a medical facility by ambulance, as deemed necessary, at my expense.

Furthermore, I hereby agree to hold harmless, indemnify, release, and forever discharge Christ’s Church of the Valley (CCV) and its staff, employees, agents, representatives, successors and assigns from and against any and all liability, claims, demands, damages, costs, expenses, legal costs, actions, and causes of action with respect to death, injury, loss, or damage to the Child, arising out of or in any way related to the Child’s participation in the named activity.

By signing this form, I grant Christ’s Church of the Valley (CCV), permission to use my name and/or my child’s name, image, likeness, or photograph in any and all forms of media. This permission is granted without time limitation and may be used in ways consistent with CCV’s mission. I am the parent or legal guardian of the minor named above and have the legal authority to execute the above release. I approve the foregoing and waive any rights in the premises.

Due to the large number of people attending this event, we are not able to ensure your picture or a picture of your child is not taken or posted on social media. If you find any pictures of yourself or your child that have been posted by CCV on our social media platforms, please email NextGen@ccv.church with a copy of the picture, the platform it was shared on, and if possible a link. We will be happy to remove the picture(s) at your request.
